Can Access to Contraception Deliver for Women’s Economic Empowerment? What We Know – and What We Must Learn

Doing Business Differently with Subnationals: Recommendations for Global Health Donors in Highly Decentralized Countries
In the big decentralized countries where global disease burden is concentrated, such as India and Indonesia, most public money for health isn’t spent by the national ministry of health, the traditional counterpart for global health funders and technical agencies. Instead, most money is programmed and spent subnationally.
Greater subnational public spending reflects growing democratization, power-sharing, and local self-determination. It also responds to the conviction that local decision-makers understand local realities better than a bureaucrat sitting in the capital city. Yet evidence on the effectiveness of subnational spending on health care and outcomes is mixed at best, and incentives for greater spending and better performance can be weak.
Given the vital importance of child vaccination programs to US national security interests, intelligence-community participation in public health services should be explicitly banned. Doing so might help restore confidence in vaccination programs—benefiting those immunized and the health and security of Americans here at home.

The President's Emergency Plan for AIDS Relief (PEPFAR) provides more than $5 billion per year to prevent and treat HIV/AIDS. Exactly how is that money spent? Donors, recipients, and even PEPFAR staff are often left guessing, because much of the extensive data the U.S. government collects on the program isn't released. In this new CGD note, Michael Bernstein and Sarah Jane Staats (Hise) urge the U.S. Congress to require that PEPFAR regularly release this data. They argue that this would improve coordination between PEPFAR and other donors, help PEPFAR staff assess progress and hold recipients accountable, and increase cost-effectiveness. Some of the data will soon be available anyway: CGD's HIV/AIDS Monitor is preparing to release PEPFAR funding data for Fiscal Years 2004-2006 obtained by a partner organization through a Freedom of Information Act request.
