As we mark the 20th anniversary of the United States President’s Emergency Plan for AIDS Relief (PEPFAR)—and the world continues to grapple with multiple global crises and mounting fiscal pressures—the unfinished HIV/AIDS agenda requires programmatic, financial, and political leadership. This is precisely why sustainability is a key pillar in PEPFAR’s new five-year strategy.  

Looking ahead, PEPFAR must address how to continue prioritizing and translating sustainability into effective programming and operational practice (while there are still HIV/AIDS programmatic goals to meet by 2030); strengthen and advance national capacity to support sustainability in the HIV/AIDS response; and leverage an HIV-specific program to support global health security, national public health institutes, and more.
